Terms of Service

By completing this contract, I agree to make a commitment to participate in the PEARLS (PFTG) program. My parent/ guardian and I know this is an exciting opportunity to be a part of a special group that will combine great talks with art, music and more. This is an important support group that I will be a part of and to help build trust in this group, I know that I need to be at every session. Three unexcused absences or inappropriate behavior are grounds for me to be asked to leave the group.

I understand we will be having important conversations about issues of interest and concern to girls my age. I also understand we will be having fun with many creative activities that will help us practice:

I hereby give permission for the participant listed above to take part in PEARLS for Teen Girls group sessions and activities, which may include virtual sessions, off-site events, academic assistance or recreational

programs. If a medical emergency arises, program staff will take all steps necessary to ensure the safety of the girl, and will call, if necessary, a public emergency vehicle for the transport to an emergency facility. I understand that I will be responsible for any transportation charges and medical expenses incurred. I agree that if a health condition exists now or in the future which would impact the participation of the girl listed above, I will contact the PEARLS staff.
